Instructions for Completing Form

On-Line: On-line registrations will only be accepted for those wishing to pay with Visa or Master Card. Complete the form below including credit card information and click on the submit button. ($50 limit per class on credit card charges).

<>Please remember our CANCELLATION and REFUND POLICY: a $50 per course deposit is required with the registration. The balance of the fee can be paid the first day of the class. The $50 deposit is non-refundable, but is transferable to register for another Communicate Institute course within one year from the date of your initial registration. The deposit cannot be transferred to another course in which you are already registered. You may find someone not already registered to take your spot and they can reimburse you for the deposit. If C.I. must cancel due to unforeseen circumstances, registrants will receive a full refund.  No Shows lose their deposit.

Although we do not anticipate having to cancel any courses, we reserve the right to do so should we not have sufficient enrollment. Specific locations and instructors are subject to change. 

Discount registrations

Substitute Discount- $30 . Must provide original letter on school letterhead from district administrator verifying your current status as a sub.

Group Discount (4 or more registering at the same time for a $30 per person discount) must be mailed in or called in at the same time (one envelope or one phone call). Do not use the on-line registration below for a group discount since the form can only accommodate one person per form. You may print out the form and make copies for each person in your group discount to complete. Have one person collect and mail in together. group discount
